Healthcare reform push continues as Minister Connor warns reserves are not a permanent fix

GREAT BAY--Minister of Public Health, Social Development and Labor (VSA) Claret Connor has acknowledged that the financial sustainability of St. Maarten’s healthcare funds remains a major concern, warning that the current situation cannot continue to be managed by allowing reserves to absorb ongoing structural deficits.

The Minister addressed questions during the 2026 Budget Debate regarding concerns over the healthcare fund’s annual shortfalls and what steps Government is taking to secure the long-term sustainability of the healthcare system.

Connor explained that the solution cannot be to continue relying on reserves to cover recurring financial gaps.

“The financial sustainability of the healthcare funds remains a significant concern and cannot be addressed by allowing the reserves to continue absorbing structural shortfalls,” the Minister stated.

He said Government’s approach focuses on two areas: strengthening healthcare financing while simultaneously controlling healthcare expenditures.

According to Connor, several measures have already been implemented or are being pursued to improve efficiency and contain costs. These include strengthening referral management, improving compliance, reducing fraud, increasing prevention efforts and improving the overall efficiency of healthcare delivery.

The Minister pointed to reforms already underway, including the increase of the SZV wage limit to Cg. 120,000 annually, which expands access to social security coverage for a larger group of employees.

He also highlighted pharmaceutical cost-containment measures, improved access to medicines, better health information and data exchange, and strengthened prevention initiatives as part of the broader effort to improve healthcare sustainability.

Connor said that a cost-containment and financial sustainability plan has also been developed by SZV, containing measures focused on operational efficiency, referral management, compliance, fraud reduction and prevention.

The Minister identified General Health Insurance (GHI) as the long-term structural reform intended to address fragmentation within the current healthcare insurance system, broaden access to coverage and create a more sustainable healthcare financing framework.

However, Connor emphasized that Government recognizes that the financial position of the healthcare funds cannot simply wait for the full implementation of GHI.

He said discussions with SZV are ongoing regarding the way forward, including cost-containment measures, the possibility of phased implementation of GHI and addressing existing funding gaps.

“The objective is not simply to preserve the reserves for as long as possible, but to move towards a situation where financing of the healthcare system is better aligned with the actual cost of providing care,” Connor explained.

The Minister noted that the specific annual deficit, current reserve position and projections regarding how long the funds can continue under current circumstances must be confirmed by SZV based on its latest financial figures.

Connor’s comments come as Government continues preparations for healthcare reform while facing increasing pressure to address rising healthcare costs and financial challenges within the existing system.

The Minister stressed that sustainable healthcare requires both improved financing structures and measures that ensure available resources are used effectively.

The healthcare debate remains closely linked to the planned introduction of GHI, which Government has identified as a major reform aimed at creating broader coverage and a more sustainable healthcare financing model.
